How It Works

Step 1: Initial Meeting & Intake

Navigator introduces program, organization completes intake form and need assessment meeting to identify service needs

Step 2: Matching & Service Planning

Organizations grouped by need, matched with vetted provider for a discovery meeting and service proposal

Step 3: Agreements & Service Launch

Navigator drafts MOU and Service Agreement. Once finalized, service delivery begins

Step 4: Ongoing Support & Evaluation

Navigator provides ongoing support, quarterly surveys, annual renewals, and feedback collection
